Electives are hands-on clinical experience and thus can never be done on B1 or B2 visa. They are more done on F1 visa than on J1 visa. The choice of visa is based on your sponsoring hospital where you will be rotating.
Once you get an elective, you will be given a visa sponsor letter by the hospital which will be the main supporting for your Visa application, interview as well at immigration check at airports. The right time to apply for visa at US Embassy is as soon as you get the letter of sponsorship from hospital or atleast 3 months before your travel date.
This time will give you advantage to book travel tickets for USA, plan your trip as well as reapply for visa if rejected.
